If you are a Settlement Class Member, you are entitled to three years of free Credit Monitoring services as part of this Settlement. You do not need to submit a Claim Form to receive Credit Monitoring benefits.

If you received an Email Notice, your Enrollment Code appears at the top of the email.

If you received a Postcard Notice, your Enrollment Code is located inside the postcard.
